Who we are

This Privacy Policy explains how CommonHealth Wellness (operated by Alphagenix, LLC) (“CommonHealth,” “we,” “us”) collects, uses, and shares information when you visit https://commonhealthrx.com (the “Site”) or use our services, including our online assessments, telehealth consultations, and medication ordering and shipping services. By using the Site you agree to this Policy.

Information we collect

Depending on how you use the Site, we may collect:

  • Contact and account information such as your name, email address, phone number, and shipping address.
  • Health information you provide in assessments, intake forms, and consultations, such as height, weight, BMI, health history, current medications, and treatment goals. Health information handled in connection with telehealth and pharmacy services is described in the Protected Health Information section below.
  • Order and payment information. Payments are processed by third-party payment processors; we do not store your full card number.
  • Communications you send us, including form submissions, emails, and text message opt-in and consent records.
  • Device and usage information such as IP address, browser type, pages viewed, referring website or advertisement, and the date and time of your visit, collected through cookies and similar technologies.

How we use your information

  • To provide, operate, and support our services, including scheduling, telehealth coordination, order processing, and shipping.
  • To communicate with you about appointments, orders, and your account, and to respond to your requests.
  • To send marketing communications where permitted by law. You can unsubscribe at any time.
  • To measure and improve the Site and our advertising.
  • To maintain security, prevent fraud, and comply with legal obligations.

SMS and Mobile Communications

If you provide your mobile phone number and opt in to receive text messages from CommonHealth Wellness (Alphagenix, LLC), we collect and use your mobile number to send you appointment reminders, scheduling links, follow-up communications, and account-related notices.

We do not share or sell mobile opt-in data or SMS consent information with third parties or affiliates for marketing or promotional purposes. Your mobile number and SMS consent status are used solely to facilitate the text messaging described above and are not shared with any third party for purposes unrelated to delivering our services, except as necessary to: send messages through our SMS service provider (Twilio), which processes messages on our behalf; comply with legal obligations, such as responding to a lawful subpoena or court order; or protect the rights, property, or safety of CommonHealth, our users, or the public.

You may opt out of SMS communications at any time by replying STOP to any message you receive, or by contacting us at support@commonhealthrx.com. Opting out of SMS does not affect your consent to other communication channels, such as email, unless you separately request removal from those as well.

Protected Health Information (PHI) and Your Privacy Rights

CommonHealth Wellness (Alphagenix, LLC) may create, receive, and maintain Protected Health Information (PHI) about you in connection with the telehealth and pharmacy-fulfillment services provided through this site. We may use and disclose your PHI for purposes of treatment (coordinating your care with prescribing providers and pharmacies), payment (processing charges for services and prescriptions), and healthcare operations (quality assessment, staff training, and administrative purposes). We may also disclose PHI where required by law, to prevent a serious threat to health or safety, or as otherwise permitted or required by the HIPAA Privacy Rule.

Your Rights Regarding Your Health Information. You have the right to: inspect and obtain a copy of your PHI; request an amendment to your PHI if you believe it is incomplete or inaccurate; request an accounting of certain disclosures of your PHI; request restrictions on certain uses or disclosures; request confidential communications by alternative means or at alternative locations; and receive a paper copy of this notice upon request. Filing a Complaint. If you believe your privacy rights have been violated, you may file a complaint with us at support@commonhealthrx.com, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.

Our Legal Duty. We are required by law to maintain the privacy of your PHI, provide you with this notice of our legal duties and privacy practices, and abide by the terms of the notice currently in effect. We reserve the right to change the terms of this notice and to make the new notice provisions effective for all PHI we maintain.

Cookies, analytics, and advertising

We and our service providers use cookies, pixels, and similar technologies to operate the Site, understand how it is used, and measure and improve our advertising. These may include Google Analytics and the Meta (Facebook) pixel. These tools may collect information such as pages viewed, device and browser data, and referral source. You can control cookies through your browser settings, opt out of Google Analytics at https://tools.google.com/dlpage/gaoptout, and manage ad preferences through your Facebook and Google account settings. Blocking cookies may affect how parts of the Site work.

Who we share your information with

We do not sell your personal information. We share information only as described in this Policy, including with:

  • Healthcare providers, telehealth platform providers, and pharmacies involved in evaluating, prescribing, dispensing, and shipping your treatment, as needed to provide care to you.
  • Service providers that help us operate the Site and our business, such as payment processing, email and text messaging, scheduling, customer relationship management, website hosting, and analytics and advertising providers. They may use your information only to perform services for us.
  • Legal and safety recipients where required by law or legal process, or to protect the rights, property, or safety of CommonHealth, our customers, or others.
  • Business transfers in connection with a merger, acquisition, or sale of assets, subject to this Policy.

How long we retain your information

We retain information for as long as needed to provide our services and to meet legal, regulatory, tax, and recordkeeping requirements. Health and prescription records, including PHI, are retained for seven (7) years. After the retention period ends, PHI is securely and permanently destroyed. Other information that is no longer needed is deleted or de-identified.

Security

We use reasonable administrative, technical, and physical safeguards to protect your information. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.

Your choices and rights

You may ask us to access, correct, or delete personal information we hold about you, and you may opt out of marketing emails (using the unsubscribe link) and text messages (by replying STOP). Some information, including health records, must be retained under applicable law, and your rights over PHI are described in the section above. Children

Our services are intended for adults 18 and older. We do not knowingly collect personal information from children under 18.

Third-party websites

The Site may link to or embed content from third-party websites. Those sites have their own privacy practices, and we are not responsible for them.

Changes to this Policy

We may update this Policy from time to time. The effective date at the top shows when it was last revised, and material changes will be posted on this page.
