This Telehealth Consent explains how care works when you use CommonHealth Wellness (“CommonHealth,” “we,” “us”). Please read it before completing an assessment or scheduling a visit. By using our site or services, you acknowledge that you have read and understood this consent.
What Telehealth Is
Telehealth means receiving medical evaluation, consultation, and treatment recommendations from a licensed provider using phone, secure video, or secure messaging instead of an in-person office visit. Clinical care for CommonHealth patients is provided by Elite Care Health, an independent physician group whose licensed providers are responsible for all clinical decisions, including whether to prescribe, what to prescribe, and any follow-up care required. CommonHealth Wellness is a technology and support platform — we do not practice medicine, and no CommonHealth employee makes prescribing decisions.
Not for Emergencies
CommonHealth is not an emergency service and cannot respond to emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room.
No Guarantee of Treatment
Completing an intake form, an assessment, or selecting a plan does not guarantee that any medication will be prescribed. Your provider will review your health history, goals, and any information you provide, and may request additional information, lab work, or a live consultation before making a treatment decision. Prescription medications are provided only when a licensed provider determines they are clinically appropriate for you.
Provider Licensure and Where We Operate
CommonHealth currently serves Kentucky residents. Your provider is licensed to practice medicine in Kentucky and will confirm your eligibility for telehealth care based on your location at the time of your visit.
Benefits and Risks of Telehealth
Telehealth offers convenient access to care without travel to an office. As with any medical evaluation, there are limits: your provider is relying on the information you provide and cannot perform a hands-on physical exam. There is a small risk that technical problems could interrupt or delay a visit, that information transmitted electronically could be intercepted despite reasonable safeguards, or that your provider may determine, based on your history, that telehealth is not appropriate for your situation and that an in-person evaluation is needed instead.
Confidentiality
Information you share during your evaluation is protected the same way it would be in an in-person visit. See our HIPAA Notice and Privacy Policy for details on how your health and personal information is collected, used, and protected.
Your Right to Withdraw Consent
Your participation in telehealth is voluntary. You may withdraw your consent to telehealth care at any time by contacting us at support@commonhealthrx.com or (859) 577-8999, without affecting your right to future care. Withdrawing consent does not cancel any pending order or billing obligation already incurred — see our Cancellation & Refunds policy for that process.
Compounded Medications
If your provider prescribes a compounded medication, you acknowledge that compounded medications are not FDA-approved. The FDA does not evaluate compounded medications for safety, effectiveness, or quality before they reach patients.
